            Clarification Text on Processing of Personal Data
            In the process of providing services to you, we would like to inform and enlighten you about the personal
                data you have provided to us, in accordance with Article 10 of the Law on the Protection of Personal
                Data No. 6698 (“KVKK”). We would like to assure you that we show the utmost sensitivity regarding the
                confidentiality and security of all personal data you provide and that all necessary technical and
                administrative security measures have been taken to protect this data.
            DATA CUSTODIAN and REPRESENTATIVE
            As Prof. Dr. Onur Egemen, in the capacity of data controller, we can process, record, store, classify,
                update, and disclose/transfer your personal data to third parties limited to the purposes described
                below, in a manner compliant with the law and the rules of honesty.
            PERSONAL DATA COLLECTED BY THE PRACTICE
            The personal data collected includes:
            
                - Identity Information: Your name, surname, Turkish ID number, passport number or
                    temporary Turkish ID number, place and date of birth, marital status, gender, insurance or patient
                    protocol number, and other identity data that can identify you.
 
                - Contact Information: Your address, phone number, email address, and other contact
                    data.
 
                - Financial Data: Your bank account number, IBAN number, credit card information,
                    billing information, and other financial data.
 
                - Health Information: Examination data, diagnosis and operation information, medical
                    history data, family medical history data, laboratory results, medical imaging results, test
                    results, examination appointment information, prescription information, medications used, pre- and
                    post-operation photos, three-dimensional visual data, clinical follow-up videos, all digital
                    communications and correspondences with your doctor during the consultation, diagnosis, treatment,
                    and follow-up process, and any personal data obtained during or as a result of the services
                    provided.
 
                - Web Browsing Data: IP address, browser information, and medical documents, surveys,
                    form information you provide with your consent.
 
            
            PURPOSE OF PROCESSING PERSONAL DATA
            Your personal data is processed based on your explicit consent and/or other processing conditions
                stipulated in KVKK Article 5/2, primarily the legal regulations we are subject to. Considering the
                services provided by Prof. Dr. Onur Egemen, your personal data may be processed for the following
                purposes:
            
                - To fulfill the requirements of the transactions related to the services provided and to ensure that
                    the relevant persons benefit from the services offered within the scope of the service provided
 
                - To fulfill our legal obligations under the Basic Law on Health Services No. 3359, the Decree Law on
                    the Organization and Duties of the Ministry of Health and its Affiliated Institutions No. 663, the
                    Regulation on Private Health Institutions Providing Outpatient Diagnosis and Treatment, the
                    Regulation on the Processing and Protection of the Confidentiality of Personal Health Data, and
                    other relevant regulations
 
                - To protect public health, conduct preventive medicine, medical diagnosis, treatment, and care
                    services, and plan and manage healthcare services and their financing
 
                - To respond to any questions or complaints you have regarding the health services you receive
 
                - To measure, increase, and investigate patient satisfaction
 
                - To invoice the services you receive
 
                - To inform you about complementary services and new services offered in addition to the services you
                    receive
 
                - To inform you about your appointment if you have made an appointment
 
                - To fulfill the information sharing, reporting, and informing obligations required by public
                    institutions and authorities as stipulated by the relevant legislation
 
                - To fulfill the information and document retention obligations arising from the legal regulations
                
 
                - To educate and inform other patients and the public, promote, increase volume, conduct scientific
                    research and education, with your explicit consent.
 
                - And other purposes stipulated in Articles 5 and 6 of Law No. 6698.
 
            
            METHOD OF COLLECTION OF PERSONAL DATA AND LEGAL REASON
            Your personal data will be collected by us for the purposes listed above and through all kinds of verbal,
                written, or electronic media (documents, contracts, printed forms in physical media; corporate website,
                email, internal software, image and call center voice recording systems in electronic media).
            We may process your personal data based on the presence of your explicit consent or one of the following
                conditions where your explicit consent is not available:
            
                - It is necessary to process personal data for the performance of a contract to which the data subject
                    is a party
 
                - It is necessary for compliance with a legal obligation to which the controller is subject
 
                - Personal data has been made public by the data subject
 
                - It is necessary for the establishment, exercise, or defense of legal claims or whenever courts are
                    acting in their judicial capacity
 
                - It is necessary for protecting the vital interests of the data subject or of another natural person
                    where the data subject is physically or legally incapable of giving consent
 
                - It is necessary to process personal data in order to perform a task carried out in the public
                    interest or in the exercise of official authority vested in the controller.
 
            
            Your special categories of personal data may also be collected, stored, and processed based on the
                following legal grounds:
            
                - Your explicit consent
 
                - If your health and sexual life data is necessary for the protection of public health, the operation
                    of preventive medicine, medical diagnosis, treatment and care services, and the planning and
                    management of healthcare services and their financing, these data may be processed by Prof. Dr. Onur
                    Egemen without your explicit consent.
 
            
            PARTIES TO WHOM PROCESSED PERSONAL DATA IS TRANSFERRED AND PURPOSE OF TRANSFER
            Your personal data may be shared with the Ministry of Health and its sub-units, the Social Security
                Institution, private hospitals with which we have agreements for operations requiring surgery, private
                insurance companies, law enforcement agencies, judicial authorities, authorized representatives,
                lawyers, tax and financial advisors, auditors, regulatory and supervisory institutions, and third
                parties providing IT support (server hosting, software, cloud computing, etc.) within the scope of the
                personal data processing conditions and purposes specified in Articles 8 and 9 of the Law No. 6698.
            Your personal data will be protected as a professional secret within the framework of the Law on the
                Protection of Personal Data No. 6698 and the relevant legislation, and except for legal obligations, it
                will not be shared with third parties/institutions/organizations without your consent.
            RIGHTS OF THE PERSON WHOSE PERSONAL DATA IS PROCESSED
            As the data subject, you have the following rights under Article 11 of the Law:
            
                - To learn whether your personal data is processed or not
 
                - To request information if your personal data has been processed
 
                - To learn the purpose of processing your personal data and whether they are used appropriately
 
                - To know the third parties to whom your personal data is transferred domestically or abroad
 
                - To request correction of your personal data if it is incomplete or inaccurately processed
 
                - To request the deletion or destruction of your personal data
 
                - To request notification of the transactions made pursuant to correction, deletion, or destruction to
                    third parties to whom your personal data has been transferred
 
                - To object to the occurrence of a result against you by analyzing the processed data exclusively
                    through automated systems
 
                - To claim compensation for the damage arising from the unlawful processing of your personal data.
                
 
            
            Your personal data that we collect and process must be accurate and up-to-date as required by Article 4
                of the Law on the Protection of Personal Data No. 6698. Therefore, if there are any changes in your
                personal data, you can notify us of your current and accurate personal information through the methods
                listed below.

            You can contact us to obtain information about the use of your personal data or to convey your

                incorrect or incomplete information.
            TEXT OF EXPLICIT CONSENT
            I hereby declare that all kinds of personal data belonging to me within the body of Prof. Dr. Onur Egemen
                are subject to the regulations stipulated under the Law on the Protection of Personal Data No. 6698 and
                other legislation, and that Prof. Dr. Onur Egemen has provided me with full and clear information
                regarding my rights within the scope of the relevant law and other legislation.
            The personal data belonging to me is collected within the scope of the service provided by Prof. Dr. Onur
                Egemen. My personal data is stored for the period required by the laws and is transferred to third
                parties to the extent required by the performance of the service agreement and the laws.
            In line with the purposes specified in the "Clarification Text available on the Website", this consent is
                for situations where the personal data processing conditions specified in Articles 5/2 and 6/3 of the
                Law on the Protection of Personal Data cannot be met and require explicit consent;
            
                - To take photos and video recordings for medical purposes before and after the operation
 
                - To process my special categories of personal data, especially health and biometric data
 
                - To process all personal data specified in the clarification text for the purpose of benefiting from
                    the services provided and to transfer them to service providers located domestically and/or abroad
                    that we receive support from (server hosting, software, cloud computing, supplier, consultant,
                    etc.).
 
            
            I give my explicit consent freely to the processing and transfer of my personal data as specified above.
